DIRECTIONS:  Please complete this form if you have an interest in serving on the LHCA Board. Terms are normally three years and it will be expected that you serve on at least one committee.  The information on this form will be presented to the LHCA membership at the annual meeting and your name will also appear on the official ballot. A LHCA board member will contact you after receipt of this form to answer any questions or concerns that you may have about Board service. Please note: Floor nominations will NOT be accepted at the annual meeting as candidates for the Board will need to complete this process. Thank you. Please print this form, fill it out and mail it to the LHCA at the address shown at the bottom of this page. Note: If you wish to nominate someone other than yourself, you must have the nominee fill out this form in order for that person to be considered.

INFORMATION:  Board meetings are held once a month from April through September, usually at 9:00 AM on a Saturday morning. The duration of the meeting is between two and three hours. You can have a significant influence on the health and vigor of Lake Hubert by being a member of the Board of Directors.
